Fields alignment in the "Test-drive" form is improper
Steps to reproduce:
1. Open the website http://hostft.kl.com.ua
2. Click on the "Тест-драйв" in the main menu
3. Pay attention to the fields alignment
Actual result: there is no fields alignment in the form "Тест-драйв"
Expected result: all fields should be aligned.
